What is included in a monthly membership?

Medical management

  • Annual Comprehensive Wellness Exams

  • Chronic disease management (ex. diabetes, high blood pressure, etc.)
  • Mental Health (Depression, Anxiety)
  • Urgent care visits (ex.  fever, injuries, etc.)

    • Cervical cancer screening, breast exams, birth control management
  • Specialty Care
    • Addiction medicine (buprenorphine and naltrexone)
    • Weight management (Medication assisted, Lifestyle medicine informed)

*Disclaimer: Until Palisade DPC has grown to a sustainable clinic, Dr. Allen will need to “moonlight” for supplemental income. Because he will need to bill Medicare for these services, he will be subject to strict regulations on Medicare billing. As such, Dr. Allen is not able to accept patients who have Medicare or are Medicare eligible until the practice is sustainable – at which point he will “opt-out” of Medicare billing and will start taking patients above the age of 65.
